Enroll in the Rely On Redemplo® Patient Support Program today!

The Rely On Redemplo Patient Support Program provides support, education, and information on financial assistance options that may be available to help you start and stay on track with Redemplo.

Enrollment

Your doctor can initiate your enrollment in the Rely On Redemplo Patient Support Program by completing the Redemplo Start Form. You will need to provide your consent to enroll in the program. Note: Enrollment in the Rely On Redemplo Patient Support Program is not required to have your prescription fulfilled.
Just follow these easy steps:

Your doctor completes and submits the form

Your doctor will access, fill out, and submit the Redemplo Start Form. The form captures all the information needed by your health plan to determine your coverage for Redemplo.

You can complete enrollment electronically

If you would like to enroll in the program, you or an authorized caregiver may sign the form to provide your consent to complete enrollment after your doctor has started your prescription for Redemplo. For online submissions, you can sign electronically with a digital signature. For fax submissions, you can sign the printed form. If you are unable to sign in person, you can complete a standalone Patient Consent Form.

REDEMPLO® (plozasiran) is an injectable prescription medicine used together with a low-fat diet to reduce triglycerides (fat in the blood) in adults with a condition that keeps the body from breaking down fats called familial chylomicronemia syndrome (FCS).

Important Safety Information

Before you start using REDEMPLO, tell your healthcare provider about all your medical conditions, including if you are pregnant or plan to become pregnant, or are breastfeeding or plan to breastfeed. It is not known if REDEMPLO could harm your unborn baby, or if it passes into your breast milk and could harm your breastfeeding baby.

Tell your healthcare provider or pharmacist about any prescription and over-the-counter medicines, vitamins, or herbal supplements you take.

What are the possible side effects of REDEMPLO?

The most common side effects of REDEMPLO include increased blood sugar levels, headache, nausea, and injection site reactions (pain, redness, or swelling).

These are not all the possible side effects of REDEMPLO. Tell your healthcare provider or treatment team if you have any side effect that bothers you or that does not go away.
